How Do I Make a Claim?
Now that you have a claim you want to send in, what should you do? We've designed a simple claims process to make sure you get your reimbursement as quickly as possible.
Step 1. Get a Claims Form
You can get an Embrace claim form either:
- that came pre-filled in an e-mail when you signed up (fastest)
- or download a claim form (fast)
- or phone us at (800) 511-9172 and we'll mail one out (slowest)
Step 2. Fill In the Claims Form
Next, fill in your and your pet's information in Part 1 and sign at the bottom of the claim form. Then have your veterinarian complete Part 2 of the claim form.
Here's a tip: store your pre-filled claims form in your pet's file at your veterinarian's office. Then you have it on hand each time you visit.
Step 3. Attach Itemized Invoices to the Claims Form
Finally, attach itemized invoices to the completed claim form and:
- fax them to Embrace at (800) 238-1042 (fastest)
- mail them to us at: Claims Dept, Embrace Pet Insurance, 781 Beta Drive, Suite D, Mayfield Village, OH, 44143 (slowest)
What Else Do I Need to Know?
Send in your claim within 60 days of your visit to the veterinarian.
You can hold onto your receipts until you've reached your annual deductible.
How Can I Get My Reimbursement Faster?
Here’s what you can do to make your reimbursement quick and easy:
- use the pre-filled form that came with your policy
- fax the claim form and attachments directly from your veterinarian's office
- get your veterinarian to fax us your pet's health history if you are lodging an illness claim
